Tooth pain is an emergency when it is severe or throbbing, wakes you up at night, radiates to the jaw, ear, or head, or comes with swelling or fever. Those patterns point to infection, nerve damage, or advanced decay rather than simple sensitivity. Pain like that rarely settles on its own and should be evaluated right away.

The usual causes are deep cavities, infected or abscessed teeth, cracked or fractured teeth, and exposed tooth nerves. In each case the nerve inside the tooth is irritated or inflamed, which produces pain out of proportion to the size of the visible problem. A dentist uses an examination and X-rays to identify which cause is responsible.

Pain sometimes fades, but that does not mean the problem is gone. When an infected nerve dies, the ache can stop even though the infection keeps spreading into the surrounding bone. That usually means diagnosing the source with an examination and imaging, then addressing it with a filling, root canal treatment, drainage of an infection, or extraction depending on the findings. Antibiotics may be used when infection has spread, but they do not replace treating the tooth.
